Steven Heffer, MD FACEP – Owner/Medical Director
Dr. Steven Heffer is a board certified Emergency Medicine physician and has been in practice for twenty years. He received his medical degree from Albert Einstein College of Medicine in New York City and completed his internship and residency at New York University Medical Center and Bellevue Hospital Trauma Center.
His past experiences include working as an attending Emergency Department (ED) physician at the University of Massachusetts where he also served as a life flight Trauma Physician. He then spent over a dozen years as an ED physician at Yale affiliated Greenwich Hospital.
Residing in Fairfield County, Dr. Heffer has been searching for a way to utilize his expertise to positively impact health services within his own community, and improve the quality of medical care available to his patients. He has already made great strides in his vision for the city of Bridgeport. Under his leadership as Medical Director and Owner, AFC of Bridgeport was granted the Business in Bloom Award in 2014 and the Regional IMPACT Health Care Award in 2015 by the Bridgeport Regional Business Council for having made significant contributions to the Bridgeport neighborhoods and surrounding towns.
Dr. Heffer is currently the National President of the Physician Advisory Committee at American Family Care (AFC Urgent Care) and a board member of the Fairfield Chamber of Commerce. He is extremely passionate and motivated to provide the highest level of medical care and service to the Fairfield County.
Waseem Shahid, MD
Dr. Waseem Shahid is a Family Medicine board certified physician with both experiences in the hospital and medical office setting. Dr. Shahid attended Government College in Pakistan where he received his Associates in Economics and Political Science and Universidad Iberoamericana in the Dominican Republic where he received his Pre-Medical and Medical Degree. He attended Texas Tech University Health Sciences Center (El Paso, TX) for his clinical training and residency in family medicine.
Dr. Shahid’s past work experience includes acting as a clinical coordinator at the Carlisle Medical Center (NY) and Gilberto Medical Center (NJ). His volunteer experiences include Kingsbrook Jewish Medical Center (NY) and Padre Billini Hospital (Dominican Republic). He also has several years of volunteer experience with voluntary clinics for Baptist Christian Health Centre (El Paso, TX) as well as a training physician for both marathons and a triathlon.
When Dr. Shahid is not devoting his time to the patients his interests include reading, medical innovations, and traveling internationally. He is also fluent in five languages: English, Spanish, Urdu, Hindi, and Punjabi. Dr. Shahid has been an essential part of the AFC team since February 2015.
Amy Smart, PA-C
Amy Smart, PA-C, graduated cum laude from the University of Florida with a Bachelor’s degree in Nutritional Sciences. She then went on to obtain her Master’s Degree in Physician Assistant Studies at the University of Florida College of Medicine.
After completing her education, she worked at Total MD Family Medicine and Urgent Care in West Palm Beach, Florida for five years. During her time in the Urgent Care at Total MD, Amy evaluated, diagnosed and treated patients from ages 4 to geriatric. In addition, she counseled and educated patients on their chronic disease management while partnering with their primary care providers.
Amy is a member of the American Academy of Physician Assistants (AAPA) and Connecticut Academy of Physician Assistants (ConnAPA). She is certified by the National Commission on Certification of Physician Assistants (NCCPA). She also holds certification in Advanced Cardiac Life Support (ACLS) and Basic Life Support (BLS).
Nicolle E. White, PA-C
Nicolle E. White, PA-C graduated from Wagner College in Staten Island, NY with a Bachelor of Science degree and completed the Master’s degree program in Physician Assistant Studies.
Nicolle has working experience in Emergency Medicine at Bridgeport Hospital, a multispecialty group in Hartford Hospital, rheumatology and subspecialty Antiphospholipid Syndrome with Dr. Thomas Greco and orthopedics at Regional Orthopedics. She holds certifications in Infectious Disease and advanced cardiac life support.
Cathleen Azarigian, APRN
Cathleen Azarigian is a board certified advanced practice registered nurse (APRN) with fifteen years of experience in the emergency room, as well as eleven years in the orthopedic and trauma department. She received her Bachelor of Science degree in Nursing from Widener University (Chester, PA) and a Master of Science degree as an Advanced Practice Nurse from Fairfield University (Fairfield, CT)
Her past experience includes work at Connecticut Access Care, an Interventional Radiology/ Ambulatory Surgery Center, and Caremark, INC. She has also worked at the Yale affiliated Greenwich Hospital in Greenwich, CT for more than twenty years, where she acted as assistant manager of orthopedics for several years. Ms. Azarigian has more than 400 hours of clinical experience that includes time in the emergency room, a general internist office, and in an OB-GYN office.
Along with fifteen years of experience, outside of the office Ms. Azarigian also spends her time as a CPR instructor. She is also an Advanced Cardiovascular Life Support (ACLS) Instructor.
She has been an integral part of the AFC team since June 2014. Ms. Azarigian is both enthusiastic and determined in providing the highest level of care and services to all her patients.
Theresa Pitassi PA-C
Theresa Pitassi is a certified physician assistant with experience in Bridgeport Hospital, St. Vincent’s Hospital, and multiple urgent care centers in Connecticut. She received her Bachelor of Arts in Biology with a Minor in Psychology from Elms College (Chicopee, MA) and a Master of Physician Assistant Studies from the University of Bridgeport (Bridgeport, CT).
Her past experience includes work with the Southbury Ambulance Association as an emergency medical technician (EMT), where she was responsible for managing soft tissue injuries, immobilizing and splinting patients, administering medicine, and assessing medical and trauma patients. Her other medical experience includes working as a Physician Assistant at MyHealth First Urgent Care and Bridgeport Emergency Department. She has had various medical training and experiences, but a few that caught her interest include trauma and obstetrician/gynecology.
Along with being CPR (cardiopulmonary resuscitation) and ACLS (advanced cardiac life support) certified, Theresa is also a Member of the American Academy of Physician Assistants (AAPA) and a Fellow Member of the Connecticut Academy of Physician Assistants (ConnAPA). Theresa always strives to provide the highest quality of medical care to the community.
